The Hanover Central Athletic Department in cooperation with Community Hospital is offering a $30 sports physical with an optional EKG (for an additional cost) to all Hanover Central High School and Middle School students for the 2015-16 school year.
Please see the attached form regarding locations and contact information to set up an appointment. A percentage of each physical charge will be donated back to the Hanover Central Athletic Department. Forms must be taken to the physical appointment and can be picked up in both the high school and middle school offices as well as online at www.hanovercentralathletics.com under the athletic forms tab.
